LookHealth Inc. (“LookHealth”, “we”, or “us”) values and respects your privacy. We are committed to safeguarding the personal information that users (the “User” or “you”) share with us through our online website and services (collectively, the “Products”). Accountability and Protection of Personal Information
LookHealth’s services include biomarker test kits, DNA test kits, and follow up consults with a LookHealth advisor. In most jurisdictions, LookHealth has overall responsibility for protecting your personal information, including personal health information. We have implemented technical, administrative and physical security measures to protect the personal information of the Users from unauthorized access and improper use. We also protect the Users’ personal information offline. Security measures are regularly reviewed and updated when required.
Although we use commercially reasonable efforts to protect your personal data and information, transmission of information over the internet is difficult to completely secure. LookHealth is not responsible for circumvention of any privacy settings or security measure contained on the website.
Collection and Use of Personal Information
Privacy legislation defines personal information as any information about an identifiable individual, and includes, but is not limited to, an individual’s name, home address, phone number, email address, date of birth, health, lifestyle or medical information, ID numbers, payment information, and bank information.

LookHealth currently uses Shopify as a third-party payment processor for the Products. When you make a purchase with LookHealth, you are also subject to Shopify’s various privacy policies and terms of use.
LookHealth also currently uses Digital Ocean to host our online website. When you access our website, you are subject to Digital Ocean’s various privacy policies and terms of use.
Our website contains links to other websites that LookHealth does not own or operate. We provide links to third-party websites as a convenience to the User. When you access the third-party websites, you are subject to their various privacy policies and terms of use.
Sharing of Personal Information
We may share your information with any companies affiliated with LookHealth, with third parties who provide services to LookHealth or to You, with other third parties for legitimate business purposes associated with the Products, or as required to comply with legal obligations.
Your personal information will be disclosed to certain third parties that provide services on our behalf, including but not limited to, laboratory testing services, hosting the website, processing payments, providing analytics, and advertising and marketing services. Our service providers are only provided with the information they need to perform their specific functions and are not authorized to use or disclose personal information for their own marketing or other purposes. We and our service providers may access, store and otherwise process personal information outside of Canada, including in the United States or other foreign jurisdictions where we or our service providers are located. LookHealth is involved in a merger, acquisition, reorganization, or sale of all or part of its business, your personal information may be transferred as part of that transaction. Consent to Collection
Upon a User creating an account with the Products or otherwise submitting their personal information to LookHealth, the User consents to the collection, use, storage and disclosure of their personal information in accordance with the terms of this Policy. If LookHealth wishes to use or disclose the User’s personal information for any purpose not set out in this Policy, LookHealth may ask for additional consent. The Users may withdraw any consent at any time by emailing or writing to us using the contact information set out below, unless withdrawal of such consent would frustrate the performance of a legal obligation.
Storage and Disclosure of Personal Information Outside of Canada
We use MySQL for the storage of certain personal information of the users. MySQL has servers distributed throughout the United States and Canada and as otherwise disclosed on the webpage of MySQL. The User’s personal information will be stored on the MySQL for the following purposes:

Data Retention

Notification of Security Breaches
Should we become aware of any data breaches where there exists a real risk of significant harm to the individual whose information has been compromised, we will, as required by law, notify the Information and Privacy Commissioner under the Freedom of Information and Protection of Privacy Act, together with any impacted individuals, as soon as feasible after the breach, so that steps may be taken to reduce any potential harm.

Subject to the exceptions set out in PIPA, each User has a right to access their own personal information that has been collected and is under our control and to request a correction for any incorrect information. The User may make a request to access or correct their personal information by contacting us at the email address set out below. The User should provide sufficient information in their request to allow us to identify the information that is being requested and the reason why it has been requested.
